North Coast Community Homes works as a team with families, caregivers and partner organizations to help our residents navigate the transition to independent living.

More than 617 individuals live in one of 193 properties located throughout Cuyahoga, Summit, Lake, Stark and Erie Counties.  The pathway to independent living begins with your county agency.

Applying to your local county Board of Developmental Disability is the first step in the eligibility process for potential placement in a NCCH home.  A Support Administrator will help to plan and identify potential North Coast Community Homes housing options.  If you are new to working with one of the county agencies, you will need to complete an Application for Services to be connected to a Support Administrator.
