North Coast Community Homes Resident Application and Lease Process

North Coast Community Homes is committed to improving quality and serving our residents and partner organizations better.  NCCH is updating all records and standardizing information collected as part of the process. Our goal is to ensure accurate and timely information that will allow us to serve the people in our properties better. 

We are making improvements to the resident application and move-in process.  Through the implementation of this, we expect to improve:

  • Safety
  • Communications
  • Community

Beginning June 1, 2023 | All resident applications will be submitted through an online application. 

How It Works

  • The Support Administrator must submit a completed application through the online portal a minimum of 30 days prior to the requested move-in date.

  • Applications will be reviewed by NCCH Residential Services staff to ensure all required information is provided.

  • Upon application review, the SA will be contacted to schedule a lease signing.

  • Lease signings will be conducted in-person and scheduled for the last Wednesday of each month.  All parties must participate in the lease signing.

  • Completed documents and signed lease must be received PRIOR to a resident moving into a NCCH property.

  • An individual CANNOT take occupancy in an NCCH property without a completed application and signed lease.

Lease Signing | What to Expect:

The lease signing is done in-person with all parties included.  Expect a 1-hour meeting.  In the event that the representatives for the resident are not able to be in person, a ZOOM connection will be coordinated for those who cannot be present in the room.

  • The NCCH Residential Services team will welcome the resident, guardian, support administrator and care provider team.
  • NCCH Residential Services staff will review the lease and discuss details, review the relations, expectations, and benefits of being an resident of an NCCH community.
  • We want to get to know the the residents and their whole team get to know NCCH.  As a non-profit organization, NCCH has services, resources and opportunities available to residents to help build a stable, safe and supportive home environment where people can thrive.
  • All documents will require signatures.
